Where did they come from? What clan was the first to use them? Who's idea was it?

it isnt just runescape that uses clan tags.

 

lots of other games do.

 

i was using cs 1.6 tags back in 03ish so yeah

No. They used [M] until a couple started using [M|Name]. They definitely didn't originate tags.
